Select from $100 to $6,000 per month (not to exceed 60% of the Insured Person's Pre-Disability Earnings). The benefits paid as a result of an Insured Person's Total Disability will be the lesser of: 1) the Monthly Benefit; 2) 70% of the Insured Person's Pre-Disability Earnings minus: a) any Other Income Benefits; and b) all other income from any employer or for any work. (If the Insured Person's Monthly Benefit would reduce to less than $50 per month due to Other Income Benefits, then the minimum Monthly Benefit under The Policy will be $50 per month.)
How This Plan Works

Under this plan, you will receive a monthly benefit beginning on the 61st, 91st or 181st day of your covered Total Disability up to age 65 if Totally Disabled before age 63. If Totally Disabled at ages 63 through 69, benefits will be payable up to 2 years, but not beyond age 70. Disability Form Series includes GBD-1000, GBD-1200, or state equivalent.

  • Why would I need disability insurance?

    If a disabling accident or illness suddenly took away your ability to work and as a result also took away your ability to earn a paycheck … how would you continue to afford the living expenses you must now pay? With the Group Disability Income Insurance Plan, your income would continue in the form of a monthly benefit you select.
  • Who is eligible for this insurance?

    Members and/or their Spouse/Domestic Partner who are citizens of the United States and Actively-at-Work (at least 30 hours per week), are under the age of 65 may apply for this coverage.

  • Do I have to meet with an insurance agent?

    Issuance of this coverage is handled over the Internet and the mail. You can review the materials in the privacy of your home and purchase your insurance directly through the mail without meeting with an agent. You can, of course, talk to a licensed representative if you’d like by contacting the toll-free number in the Contacts section.
  • How do I apply?

    Download a no-obligation application and brochure containing detailed plan information and plan provisions, including economical costs, exclusions, limitations and terms of coverage.
  • How much insurance can I apply for through this program?

    Select from $100 to $6,000 per month (not to exceed 60% of the Insured Person's Pre-Disability Earnings). Any recurrent Total Disabilities are also covered for full benefit periods, if each disability period is separated by less than 3 months during which the insured is Actively at Work, or the later Total Disability is caused by an unrelated cause.

     

    The benefits paid as a result of an Insured Person's Total Disability will be the lesser of: 1) the Monthly Benefit; 2) 70% of the Insured Person's Pre-Disability Earnings minus: a) any Other Income Benefits; and b) all other income from any employer or for any work. (If the Insured Person's Monthly Benefit would reduce to less than $50 per month due to Other Income Benefits, then the minimum Monthly Benefit under The Policy will be $50 per month.)

  • How much will this coverage cost?

    Your premium is based on your age, the monthly disability benefit you request plus the Elimination Period. Refer to the rate chart in the attached brochure for important information, including how costs increase as you age.
  • What if I have second thoughts after I apply?

    Once approved, you will be sent a Certificate of Insurance. You will have 30 days from the date of receipt to review the certificate. If you are not satisfied with the terms of the certificate, simply return it to the Insurance Administrator, and any premiums paid will be refunded minus any claims paid. Your coverage will then be invalidated.
  • What is the Elimination Period?

    The Elimination Period is a specified amount of time (for example, 60 days) you must be totally disabled before benefits can be paid. You select an Elimination Period when you apply for your disability insurance coverage. You can choose from the following Elimination Periods: 60 days, 90 days or 180 days.
